How We Conducted This Study
This data brief analyzes state-by-state trends in private sector employer health insurance premiums and deductibles for the under-65 population from 2010 to 2019.
The data on total insurance costs, employee premium contributions, and deductibles come from the federal Agency for Healthcare Research and Qualitys annual survey of employers, conducted for the insurance component of the Medical Expenditure Panel Survey . The MEPSIC is administered to workplace establishments. Establishments represent a work location, not necessarily a firm, which can employ people in many locations. Workplace establishments are selected each year from the Census Bureaus Business Register a confidential list of such establishments in the United States. Once selected, establishments are contacted via mail and phone to establish a contact person who is knowledgeable about the health insurance benefits offered to employees. This contact is asked about each of the health plans offered to employees that work at the establishment location. If the establishment offers more than four plans, details are collected about the four plans with the largest enrollment. In 2019, MEPSIC surveyed 40,451 establishments and had a response rate of 59.2 percent. The total number of surveys sent in 2019 was similar to prior years, but there was a lower response rate.

Prescription drugs are also typically covered even if you havenât met the deductible. However, certain plans may require a deductible for prescription drugs, before insurance helps to shoulder the costs.

Under Obamacare, many preventive medical services may not apply to the annual deductible. This is a good thing, since it enables people to get preventive medical care at no out-of-pocket cost, even if their deductible amount has not yet been paid in full. Are Hdhps Leading To Lower Health Costs
A key tenet of an HDHP is that it can reduce health care costs — both for employers and employees in lower premiums.;
However, our Insure survey found that the vast majority of people with a;high deductible;plan havent seen lower health care costs. Forty-six percent of people surveyed said their costs increased and 41% said they stayed the same. A mere 13% said their costs decreased. These findings were nearly identical to 2019’s results when 46% said costs increased, 32% said they remained the same and 11% said they decreased.;
One drawback with an HDHP is that paying more for doctor appointments and tests may make people delay care.
The Insure.com survey found that concern is a reality. Nearly half of people with an HDHP said theyve delayed care because of cost:;
- 40% said they put off care. This was down from 56% in 2019.
- 55% said they havent delayed care.;
- 5% weren’t sure.
Thats a problem not just for the plan members, but the employer and health insurer, too. Delaying necessary care could lead to more health problems and higher health bills later.;
Another concern is that people with;high-deductible;plans often say their plans arent educating them to improve health care decisions. High-deductible health plans were once commonly called consumer-directed health plans. A vital piece of those plans is educating patients to help them become better health care consumers.
What Does A Disappearing Deductible Mean
A disappearing, or vanishing, deductible is an endorsement that can be added to your insurance to lower your costs. For each year you do not file, your payment will be reduced by 20%. Ideally, after five years of driving, there would be no fee to pay unless you make a claim.
- It is an extra way to save.
- You pay less money out of pocket if you file an at-fault accident.
- Once you sign up, your insurer will automatically lower your cost each year.
- You will have to pay an extra cost to add it to your plan.
- Not all drivers qualify.
- Not all insurers offer this incentive.
- It may or may not be worth it, depending on when/if you file.
Digging Deeper For Pricing Information
For more details, we consulted the 2020 Health Insurance Exchange Premium Landscape Issue Brief linked to the bottom of the press release. It reveals that 27-year-olds buying silver plans will see their premiums increase by 10% or more in Indiana, Louisiana, and New Jersey.
More importantly, it reveals that the percentage changes don’t tell us much about what people are actually paying: “Some of the states with the largest decreases still have relatively high premiums and vice versa,” the brief states. “For example, while Nebraskas benchmark plan premium decreased 15% from PY19 to PY20, the average 27-year-old PY20 benchmark plan premium is $583. On the other hand, while Indianas average PY20 benchmark plan premium increased 13% from PY19, the average 27-year-old PY20 benchmark plan premium is $314.”
In fact, the benchmark plan premium for a 27-year-old in 2020 is a whopping $723 in Wyoming. How many 27-year-olds can afford that kind of monthly premium? By contrast, New Mexicos 2020 benchmark plan premium for a 27-year-old is the lowest in the nation at $282.
All of these numbers only apply to the 38 states whose residents buy plans through the federal exchange at Healthcare.gov. Residents of California, Colorado, Connecticut, Idaho, Maryland, Massachusetts, Minnesota, Nevada, New York, Rhode Island, Vermont, Washington, and Washington, D.C. buy insurance through their state’s exchange.

Deductibles can vary significantly from plan to plan. According to the Kaiser Family Foundation , the 2020 average deductible for individual, employer-provided coverage was $1,644 . KFF reported the average 2021 deductible for marketplace plans sold via HealthCare.gov, by metal rating: $6,921 for Bronze plans, $4,816 for Silver plans, $1,641 for Gold plans, and $0 for Platinum plans.
